Member information may be shared within the staff of Patient Navigator LLC if needed to manage your account.  We will not disclose your personally identifiable information to third parties except to the extent necessary to provide you with a requested product or service (for example communicating with a medical provider on your behalf).   You will be asked to sign a release form authorizing any communication we may need to have with third parties.  Your health care providers or insurers will likely ask you to sign a similar release.  We will never make available your information to any third-party marketing services.

We assist members by evaluating and prioritizing problems they face in the healthcare delivery system, educating members about options and resources, and, when a member chooses to purchase personal navigator hours for one-on-one advocacy services, we develop a plan to resolve the problem(s), procure the information desired by the member and/or, when applicable, implement the solution. The member is kept informed throughout this process by telephone, email or in person. Our personal advocacy services often require us to work with third-parties on the member’s behalf, such as insurance companies, healthcare professionals or service providers.

When the paid services of a personal patient navigator are purchased through this website, a separate, written privacy form will be provided to you and a release from you allowing Patient Navigator LLC to speak with other providers on your behalf may be required. Individual medical providers usually also have their own release forms which you may be required to sign so that we may speak about you with those providers. Patient Navigator LLC is not a covered entity under the laws of the Health Accountability and Portability Act (HIPAA) because we do not create confidential medical information. However, Patient Navigator, LLC abides by and enforces the rules governing protection, storage and security of confidential patient medical information.

To the extent you are notified of uses of your personal information which are different from those described in this Policy Statement during your enrollment in any particular service, or program, the uses of which you are specifically notified will govern that service or program (for example the miCard® program or the TherapEase Cuisine program).
